Qualifying Government Assistance Programs
The programs listed below are examples of some of the qualifying programs. Qualifications can vary by state. Check your states qualifications on the enrollment form.

Qualifying Customers
If you are currently enrolled in at least one of the above programs, you
may be qualified for Lifeline government assisted home telephone service.
To get the discounted government assistance rate, we will need a signed self certification form from
you and a copy of a docoument showing your enrollment in a
qualifying assistance program, or proof of income (for
those who are qualifing based on income). If you applied for service on-line, you have already
completed the signed Lifeline certification. If you applied over the phone, follow the "lifeline forms"
tab above to submit a Lifeline form.
Texas Customers
Resedients in Texas must fill out the
Lite-up Texas application on the Texas Public Utilities Commision website. The Texas PUC will then provide that
application to the Texas LIDA (Solix). The Texas LIDA will then give their approval to us to give you the
Lifeline discount on your telephone service.
