Submit Lifeline Eligibility Proof
The FCC's reform of the Lifeline program requires that all Lifeline assistance applicants provide proof of eligibility. Choose the appropriate type of proof based on your qualification method. Proof and methods for getting that proof to us are outlined below.
For those qualifying based on participation in a government assistance program, (like Veterans Pension,
Food Stamps (SNAP), Medicaid, etc.) you will need to provide proof that you are in that program.

For those qualifying based on household income level, you must provice proof of income. Choose a form of proof from the "Proof of Income" column below.
Please be sure to include your name and new account number with all proof document submissions.
