Submit Lifeline Eligibility Proof
The FCC's reform of the Lifeline program requires that all Lifeline assistance applicants provide proof of eligibility. Choose the appropriate type of proof based on your qualification method. Proof and methods for getting that proof to us are outlined below.

| Please be sure to include your name and new account number
with all proof document submissions. Write the information at the top of Fax documents. For email and text, include the information in the body.

EBT On-Line Account Screen Capture for proof of eligibility.
If you have an EBT card, you can log into your EBT account on-line and capture a picture of your account
status screen showing your name, account number, and account status. Send us that screen shot for
proof of eligibility! Just click your state below to go to the EBT account login. If you have EBT but don't
have an website login, you can create one at the site.
To capture the picture of the screen, go to the account status screen showing your name, account number, and
account status. Then, hold down the ALT key and press the "Print Screen" key. This will capture the picture of the
screen to your clipboard. Now, just paste that in an email to us by right-clicking in the body of the email and
clicking "paste". Or, paste it into a Word document and send that to us.
